An organization organizing health fund grants for Worcester County asked me to get the word out to our town.
The Greater Worcester Community Foundation is accepting applications for its Fallon/OrNda Community Health Fund grants.
Grants range from $5,000 – $25,000. Grants should be used for:
- supporting or creating primary care outreach services to vulnerable populations
- developing continuing managed care services (rather than episodic or uncoordinated care)
- removing barriers that prevent people from receiving services (e.g., lack of transportation, cultural competency of providers, language differences)
- disease prevention and health promotion programming
- and/or agency collaboration
Grants may be for operational expenditures such as personnel costs, program expenses, small equipment purchases or physical improvements. Collaborative efforts will be given a higher priority.
Qualified applicants from any Worcester County community can apply by Tuesday, February 17.
